Acute Myocardial Infarction

 

AMI Checklist

INPATIENT

  • ASA within first 24 hours (PO or Rectal: NPO is not a valid reason for holding)
  • Care Plan - General - Acute Coronary Syndrome
  • Care Plan - Specific - Coronary Angiogram with Stents Care After (if applicable)
  • ACS Education - Document EVERY shift (ACS education is automatically added when you add the ACS Care Plan)
  • Cardiac Rehab Phase 1 or 2 Referral - Request referral from primary provider or cardiologist for appropriate patients
  • If uncertain, provider may place referral for Cardiac Rehab RN to evaluate​

DISCHARGE MEDICATIONS

  • If any of the following are not ordered, inform the provider unless provider has documented contraindication
  • Aspirin
  • Beta blocker
  • ACE/ARB/ARNI for EF < 40%
  • High intensity statin
  • P2Y12 inhibitor (Clopidogrel, Ticagrelor)

DISCHARGE ORDERS

  • If any of the following are not ordered, inform the provider unless provider has documented contraindication
  • Cardiac Rehab Phase II

DISCHARGE AVS

  • Acute Coronary Syndrome: Bold the risk factors pertinent to the patient
  • May also use Heart Attack​
  • Diet: Heart Healthy Diet or other as specific to the patient
  • ASA and Your Heart: Include if patient is on aspirin (ASA)
  • Smoking Cessation: Include if the patient has smoked within the past 12 months
  • Cardiac Related/New Medications: Include if the patient is discharged on any new medications or cardiac medications, such as: ASA, statin, beta blocker, ACEI/ARB/ARNI if EF < 40%, Plavix, Brilinta, Coumadin or another anticoagulant, Aldactone, Nitroglycerin, etc
  • Cardiac Surgery: Include if applicable to the patient
  • Scheduled Appointment: Provide date for follow-up with Cardiology OR order for ambulatory referral to Cardiology and instructions for the patient regarding appointment
  • Cardiac Rehab Phase II: Include if the patient is a candidate
